How do I pay for my visit?
We collect any co-pay, co-insurance or portion of your deductible owed at the appointment. We accept cash, check, Visa, Mastercard, Discover, American Express

Can we function as Primary Care?
We do not function as Primary care. All patients are expected to maintain a relationship with a PCP.

What about a patient portal?
We use Patient Fusion for our portal. Please ask for access if you have not established an account after your 1st appointment. It is a quick and easy way to communicate with the providers, office staff, see your lab results, appointments, etc.

What about refills?
Please have ALL prescription refill requests sent electronically from your pharmacy.
Some prescriptions require routine lab testing and office visits. Refills may not be approved if you miss your appointment and don't have necessary blood work drawn. This is for your safety.

What are our fees?
There is a $50.00 charge for all No-Shows or appointments cancelled with less than 24 hours notice.
There is a minimum fee of $35 to fill out any forms.
We refer any patient seeking disability to see their PCP. We will comply with records requests but will not initiate disabilty paperwork.
